October 2, 2013 News Review From Harvard Medical School -- More Details in Long-Term Hormone Study Results of a new, longer-term study confirm that women should not take hormone replacement therapy to reduce any disease risk after menopause. But short-term treatment for menopause symptoms seems to be safe. The new results are consistent with advice doctors have been giving for about a decade. The new study is a follow-up to the Women's Health Initiative. That study changed medical practice when it was published in 2002. The study was stopped early when it found that women who were randomly assigned to take estrogen and progestin had higher risks of breast cancer, heart attack, stroke and blood clots. They were compared with women taking placebo (fake) pills. A study of women taking estrogen alone found a slightly higher risk of blood clots and stroke. The new study looked at the women's health after 13 years. It clarified some risks for younger women in the study, those ages 50 to 59. Younger women who were in the estrogen-only study had a slightly lower rate of heart attack and fewer deaths than women who took placebo pills. New Analysis Confirms Hormone Therapy Won't Prevent Disease After Menopause

By Amy Norton HealthDay Reporter TUESDAY, Oct. 1 (HealthDay News) -- No good evidence supports using hormone replacement therapy after menopause to prevent heart disease or other chronic ills, but short-term use for hot flashes should be an option. That's the conclusion of the latest analysis of the Women's Health Initiative (WHI), a group of major U.S. trials set up to test use of hormone replacement therapy for preventing chronic diseases in healthy, older women. The WHI was launched in the 1990s, at a time when doctors were prescribing hormone replacement therapy to postmenopausal women to ward off heart disease. The results changed common practice, however. In 2002, one of the trials was stopped early when researchers found that women taking the hormones -- estrogen-plus-progestin pills -- actually had higher risks of blood clots, heart attack, stroke and breast cancer than placebo users did. A second trial, looking at estrogen therapy alone, was stopped two years later. Women on the hormone showed no lower risk of heart disease, but did have a slightly elevated rate of blood clots and stroke. Review adds to hormone replacement therapy's risks, benefits

A follow-up to one of the largest studies on postmenopausal women reveals that hormone replacement therapy (HRT) may not provide protective benefits against heart disease and other chronic conditions, and some forms might raise risk for certain diseases, like breast cancer. However, short-term hormone use for menopause-related conditions may still be beneficial. The new study, published Oct. 2 in JAMA, re-examined women from the Women's Health Care Initiative, a research program started in 1991 that involved nearly 162,000 postmenopausal women. The new analysis included data on the women 13 years after the landmark study was completed. During the initial study, the women either took a placebo, estrogen, or an estrogen and progestin combination treatment for five to seven years. Previously, doctors believed that hormonal treatments that use estrogen may be able to treat menopausal symptoms such as hot flashes and vaginal dryness, as well as provide protection against some conditions caused by aging like heart disease and bone loss. Janice (who asked that her full name not be used for privacy reasons) had heard this treatment might be dangerous to her heart, and worried about risking her health. Its a concern many American women have shared over the past decade since the benefits of hormone replacement therapy have been called into question. A large study called the Womens Health Initiative (WHI) was instrumental in casting doubt on these hormones. Tuesday, scientists from the WHI released what they say is the definitive study on the safety of hormone replacement therapy (HRT). The bottom line: Its OK for most healthy women who have just entered menopause to take hormones for a short period of time, but the researchers do not recommend it for long-term use. Hormone replacement therapy less risky for women in their 50s, latest data suggest

In the decade since a landmark study uncovered the risks of taking hormone replacement therapy for disease prevention, some doctors have shied away from prescribing the drugs even for women with severe hot flashes and other menopausal symptoms. But the latest followup data from the Womens Health Initiative trial involving 27,000 postmenopausal women suggest that the benefits of symptom relief outweigh the risksat least for women in their 50s. Among women between 50 and 59, increased risks associated with taking a combination of estrogen and progesteronesuch as strokes, blood clots, and breast cancerwere very small, less than 1 percent during the five years they were taking the hormones, according to the research published Tuesday in the Journal of the American Medical Association. Women in the trial who were still experiencing menopausal symptoms had significant improvement in these symptoms when taking hormone therapy compared to a placebo and had a very low risk of adverse events, said Dr. JoAnn Manson, principal investigator of the Womens Health Initiative and chief of preventive medicine at Brigham and Womens Hospital. Synthetic Estrogens Pose Greater Clot Risk Than Natural Forms of Hormone

A Mckeone Carolyn / Getty Images In a head-to-head comparison of two major forms of hormone replacement therapy, a more natural version of estrogen proved less dangerous to the heart than a synthetic one made from the urine of pregnant mares. While previous studies linked hormone therapy to a higher risk of breast cancer and heart disease, few looked at whether different forms of the hormone made a difference in the magnitude of these risks. So scientists from the University of Washington, Seattle studied384 postmenopausalwomen between theages of 30 and 79 who were using either conjugated equine estrogens (CEEs), or estradiol a natural, bioequivalent form of the hormone. After monitoring the women for vascular events such as blood clots, heart attacks, and strokes between January 2003 and December 2009, they found that 68 women developed blood clots, 67 women had a heart attack and 48 women experienced a clot-related stroke. Only the risk of blood clots was statistically significant, but the researchers say that the type of estrogen pill a post-menopausal women chooses may affect her circulation and risk of vessel disorders. It said the company--which was now operating under another name--was closed. The company marketed its bioidentical hormone replacement therapy as safe and natural; a way for menopausal women to improve their sex drive and combat fatigue by having their hormones replaced by having tiny pellets placed under their skin. But its what the company doesnt tell you, says the FDA and many doctors, that can lead to uncommon but serious risks. "For estrogen therapy, the most serious risk is death," said Dr. Mark Dykowski, an OBGYN with Beaumont Hospital. "Estrogen has a big black box warning for the FDA, that says it can cause breast and uterine cancer.
