Advanced Heart Failure Recovery Program comes to Deptford Center for Rehabilitation and Healthcare

The Deptford Center for Rehabilitation and Healthcare and Lourdes Cardiology have launched a partnership designed to improve the quality of life and reduce readmission rates for congestive heart failure (CHF) patients in the South Jersey area. Working together, the Deptford Center staff and Lourdes Cardiology team will help CHF patients manage their condition under carefully developed and supervised protocols. This partnership pairs the Cardiac Recovery program at the Deptford Center with experts from the Lourdes Cardiology Heart Failure Center, the regions largest and most experienced team.

All of our staff involved with our Cardiac Recovery program have successfully demonstrated, through a rigorous training program and examination, that they possess the ability to understand and clinically manage patients with heart failure. Our expectation as partners with Lourdes Cardiology is to elevate the standard of care necessary to address the growing population suffering from heart failure and to create a new standard for recovery, added Rozenberg.

Ginny Trickett, Director of Nursing at the Deptford Center, together with the education team at Centers Healthcare and Dr. Horwitz, designed and coordinated the facility-wide clinical training efforts necessary to drive the Cardiac Recovery program forward.

Effectively treating someone with heart failure requires a holistic approach. Its about addressing the whole person, from providing a low-sodium, heart healthy diet and managing their daily physical therapy routine, to assisting them when they return to their own homes, said Trickett. For example, the team would ensure that patients have access to a scale so they can weigh themselves daily. Any overnight increase of just a few pounds could be an early warning of an impending problem that could be addressed immediately through a change in medication or diet via an outpatient visit.

All of Deptfords direct care clinical staff have received specific Centers Certification in heart failure and Cardiac Recovery.

Deptford has demonstrated their ability to manage high-acuity patients. They are committed to working with our team to aggressively manage CHF and to reduce hospital readmissions, added Dr. Horwitz.
