By Tim Lueckenhoff/President, Association of Boxing Commissions In July 2011, the ABCs medical committee, formed under my tenure, published a Handbook of Ringside Medicine. This handbook established guidelines for dealing with TRT and TUEs. At this 2011 annual convention, this publication was distributed to the attending members. In addition, a very detailed presentation was provided by the ABC Medical Chair Dr. Sherry Wulkan on the issue of TRT TUEs in both 2011 and 2012. Since 2011, the ABC medical committee has suggested that its members follow its TUE protocols recommended by the ABC medical committee and modeled upon international Olympic standards. At this point, the ABC will continue to recommend its members follow these extremely tough therapeutic use exemption standards. These standards, in place since 2011, would allow TUEs for TRT in only the rarest of cases to a very select group of contestants who have a specified legitimate medical need. I note that the Olympics has seen fit to grant a TRT TUE for more than one individual, including for an individual who lost his testicles in an accident. In a stunning reversal of policy, the Nevada State Athletic Commission today voted unanimously to ban therapeutic-use exemptions for testosterone-replacement therapy. Moving forward, all fighters who wish to receive permission to undergo hormone replacement will not be allowed to apply for the exemption, and previous applicants will not be allowed another one. NSAC commissioner Raymond Skip Avansino proposed the ban and said the commission does not have the resources to effectively monitor exemptions. NSAC chairman Francisco Aguilar added the commissions previous exemption process took up too many resources for too little payoff. Commissioners Avansino and Pat Lundvall also ventured that the NSAC is under no obligation to honor exemptions issued by other states. The UFC on Thursday released an official statement supporting the ban not long after the NSAC made its ruling. The Ultimate Fighting Championship fully supports the decision made today by the Nevada State Athletic Commission regarding the immediate termination of therapeutic use exemptions (TUE) for testosterone replacement therapy (TRT), the statement read. The Nevada State Athletic Commission voted unanimously to ban the use of testosterone replacement therapy (TRT), on Thursday. TRT is a hormone treatment, usually in the form of an injection or cream, used to offset the symptoms of hypogonadism (which in men includes fatigue, erectile dysfunction, declining muscle mass, etc.). However, one of the chief causes of hypogonadism is prior steroid use. In the past, fighters such as Chael Sonnen, Dan Henderson, Frank Mir and Forest Griffin, have been granted therapy use exemption (TUE) by the Nevada State Athletic Commission. Those are now off the table, with the commission no longer recognizing TUEs, including those issued outside of Nevada. Vitor Belfort, who has received TRT exemptions in the past (he also tested positive for anabolic steroids in 2006), is one of the first fighters affected, as he challenges Chris Weidman for the UFC middleweight title at UFC 173 in Las Vegas. A recent episode of Outside the Lines highlighted the fact that MMA fighters had a much higher rate of receiving TUEs than athletes in other sports.
