Hormone replacement therapy (HRT) for transgender and transsexual people changes the balance of sex hormones in their bodies. Some intersex people also receive HRT, either starting in childhood to confirm the sex to which they were assigned, or later, if this assignment has proven to be incorrect. Its purpose is to cause the development of the secondary sex characteristics of the desired sex. It cannot undo many of the changes produced by the first natural occurring puberty, which may necessitate surgery and/or epilation (see below). The requirements for hormone replacement therapy vary immensely, often at least a certain time of psychological counseling is required. Some organizations still require a period of time living as the desired gender role, based on standards such as the Standards of Care for the Health of Transsexual, Transgender, and Gender Nonconforming People. This period is sometimes called the Real Life Experience (RLE). Doctors generally use the term "perimenopause" to refer to the time period beginning prior to the menopause (when some of the signs and symptoms of menopause begin to occur) up through the first year following menopause. Menopause itself is defined as having had 12 consecutive months without a menstrual period. Menopause symptoms begin gradually while the ovaries are still functioning and a woman is still having menstrual periods. These symptoms can begin as early as the 4th decade of life (when a woman is in her 30s) and may persist for years until menopause has occurred. The symptoms occur early because the levels of hormones produced by the ovaries (estrogen and progesterone) decline slowly over time, explaining why pregnancy is still possible, but less likely to occur, as a woman reaches her forties. Hormone replacement therapy (HRT) in menopause is medical treatment in surgically menopausal, perimenopausal and postmenopausal women. Its goal is to mitigate discomfort caused by diminished circulating estrogen and progesterone hormones in menopause. Combination HRT is often recommended as it decreases the amount of endometrial hyperplasia and cancer associated with unopposed estrogen therapy.[1][2] The main hormones involved are estrogen, progesterone and progestin. Some recent therapies include the use of androgens as well.[3] The 2002 Women's Health Initiative of the National Institutes of Health found disparate results for all cause mortality with hormone replacement, finding it to be lower when HRT was begun earlier, between age 50-59, but higher when begun after age 60. In older patients, there was an increased incidence of breast cancer, heart attacks and stroke, although a reduced incidence of colorectal cancer and bone fracture.[4] Some of the WHI findings were again found in a larger national study done in the UK, known as The Million Women Study. Hypoparathyroidism refers to a rare endocrine disorder which results in the human body producing inadequate levels of parathyroid hormone.
