IVF treatment is tailored to your specific needs by your fertility specialist. Most patients who undergo IVF will be prescribed one of two main treatment protocols Long Down Regulation and Antagonist treatment cycles. Long down regulation treatment is the process of suppressing a womans natural hormones before fertility medications. Approximately 3 weeks after your period starts you will have a blood test and begin pre-IVF treatment [GnRH analogue] in the form of a Synarel nasal spray or Lucrin injection to control your natural hormones before the fertility medication starts. About 12 days later another blood test will be taken to make sure your own hormones are low [or down regulated] and you will be instructed when to begin daily hormone Follicle Stimulating Hormone injections [Gonal-F or Puregon]. During your normal menstrual cycle you release one egg per month. Your odds of getting pregnant are better with more eggs and using fertility drugs increases the number of mature eggs that are released. It's recommended that IVF be offered with fertility drugs to stimulate your ovaries as you have a better chance of pregnancy as a result (NCCWCH 2013: 329). It was developed more than 30 years ago for the treatment of women with damaged Fallopian tubes, and this remains an important reason for treatment today. How does IVF work? All IVF treatments begin with a course of hormone therapy to stimulate the development of several follicles in the ovary. These are collected as eggs, which are then fertilised in a test-tube ('in vitro') to create several embryos. After between two and five days in an incubator, one or two of these embryos are transferred through the vagina to the uterus, where implantation occurs and pregnancy begins. However, in IVF as in natural conception, not every embryo implants to become a pregnancy, which is why surplus embryos are frozen - so that a subsequent transfer might be tried if the first one fails.
