Cardiac resynchronization therapy – Wikipedia

An implanted cardiac resynchronization device is a medical device used in cardiac resynchronization therapy (CRT). It resynchronizes the contractions of the hearts ventricles by sending tiny electrical impulses to the heart muscle, which can help the heart pump blood throughout the body more efficiently. CRT defibrillators (CRT-D) also incorporate the additional function of an implantable cardioverter-defibrillator, to quickly terminate an abnormally fast, life-threatening heart rhythm. CRT and CRT-D have become increasingly important therapeutic options for patients with moderate and severe heart failure.[1] Cardiac resynchronization therapy (CRT) is an effective therapy in patients with heart failure and dyssynchrony identified as a prolonged QRS duration.[2] Benefits include improved exercise tolerance, reduced remodeling (reduction in LV chamber size and measures of mitral regurgitation), reduced mortality, and reduced need for hospitalization in patients in sinus rhythm. Several research papers[3][4] have proposed software platforms for planning and guiding the implantation of CRT devices. What happens in rehabilitation?Similar tothe acute care facility, the TBI patient will be cared for by a team of professionals who specialize in the care of trauma victims. Their goals are to: Each day, the patient will participate in therapy. Initially, the patient may require staff assistance for even the most simple activities: brushing teeth, getting out of bed and eating. Every year, an estimated 1.5 million individuals sustain a traumatic brain injury (TBI), and approximately 75% of these are classified as a mild TBI [1,2]. The American Congress of Rehabilitation Medicine defines a mild traumatic brain injury as a patient who has had a traumatically induced physiological disruption of brain function, as manifested by at least one of the following: but where the severity of the injury does not exceed the following: loss of consciousness approximately 30 minutes or less; after 30 minutes, an initial Glasgow Coma Scale (GCS) of 13-15; and post-traumatic amnesia (PTA) not greater than 24 hours.[3] Mild TBI has been termed a silent epidemic, [4] because many patients do not have visible physical signs. Rather, many patients possess disabling cognitive, psychological, and/or behavioral impairments and employment disabilities that are often unnoticed or misdiagnosed. A ventricular septal defect (VSD) is a defect in the ventricular septum, the wall dividing the left and right ventricles of the heart. The extent of the opening may vary from pin size to complete absence of the ventricular septum, creating one common ventricle. The ventricular septum consists of an inferior muscular and superior membranous portion and is extensively innervated with conducting cardiomyocytes. The membranous portion, which is close to the atrioventricular node, is most commonly affected in adults and older children in the United States.[1] It is also the type that will most commonly require surgical intervention, comprising over 80% of cases.[2] Membranous ventricular septal defects are more common than muscular ventricular septal defects, and are the most common congenital cardiac anomaly.[3] Ventricular septal defect is usually symptomless at birth. It usually manifests a few weeks after birth. VSD is an acyanotic congenital heart defect, aka a left-to-right shunt, so there are no signs of cyanosis in the early stage. However, uncorrected VSD can increase pulmonary resistance leading to the reversal of the shunt and corresponding cyanosis.
