Instead, a diagnosis of multiple sclerosis often relies on ruling out other conditions that might produce similar signs and symptoms, known as a differential diagnosis. Your doctor is likely to start with a thorough medical history and examination. Your doctor may then recommend: In most people with relapsing-remitting MS, the diagnosis is fairly straightforward and based on a pattern of symptoms consistent with the disease and confirmed by brain imaging scans, such as MRI. Diagnosing MS can be more difficult in persons with unusual symptoms or progressive disease. In these cases, further testing with spinal fluid analysis, evoked potentials and additional imaging may be needed. There is no cure for multiple sclerosis. Treatment typically focuses on speeding recovery from attacks, slowing the progression of the disease and managing MS symptoms. Some people have such mild symptoms that no treatment is necessary. For primary-progressive MS, ocrelizumab (Ocrevus) is the only FDA-approved disease-modifying therapy. It slows worsening of disability in people with this type of MS. Cerebral palsy (CP) is a broad term that describes a group of neurological (brain) disorders. It is a lifelong condition that affects the communication between the brain and the muscles, causing a permanent state of uncoordinated movement and posturing. CP may result from several problems, such as lack of oxygen to the brain, genetic conditions, infections, brain hemorrhage, severe cases of jaundice, and injury to the head. Many cases of CP have unknown causes. The disorder occurs when there is abnormal development or damage to areas in the brain that control motor function. It occurs in approximatelythree out of every 1,000 live births. Risk factors for CP include the following: Prematurity Blood clotting disorders Very low birthweight (especially in babies weighing less than3.3 lbs.) With the help of hypophysis (or pituitary gland), the hypothalamus develops a connection between endocrine system and nervous system in the body. Parkinsons disease (PD) can be challenging to diagnose, particularly in early stages of the disease, and people who are suspected to have PD should be seen by a neurologist specially trained in movement disorders. Diagnosis of PD is generally made using a medical history and a physical exam or a neurological exam.1 Doctors use rating scales to assess the stage of PD in an individual. The two most commonly used rating scales are the Hoehn and Yahr scale and the Unified Parkinsons Disease Rating Scale (UPDRS). The Hoehn and Yahr scale describes five stages to PD progression, while the UPDRS has four parts with multiple points in each section that are individually scored.2,3 Treatments for PD are more effective in early stages of the disease, so early detection of PD is important. In addition, physical therapy and exercise, which greatly improve symptoms and delay progression of the disease, are much easier to perform in the early stages. One of the active areas of research in PD is the search for biomarkers, clinical markers in the blood, urine, or cerebral spinal fluid that reliably detect PD. Dr. McCulloch is a Principal Investigator of the National Biomedical Computation Resource and the Cardiac Atlas project, and Co-Director of the Cardiac Biomedical Science and Engineering Center at UCSD. He served as Vice Chair of the Bioengineering Department from 2002 to 2005 and Chair from 2005 to 2008. Dr. McCulloch is Director of the UCSD Interfaces Graduate Training Program and the Interdisciplinary Ph.D. Specialization in Multi-Scale Biology.
