Current Research We are currently engaged in optimization of culture conditions of human bone marrow derived mesenchymal (stromal) stem cells for their expansion and differentiation into cartilage forming cells for the future repair of cartilage defects occurring in osteoarthritis and rheumatoid arthritis. A proprietary system is being explored for the delivery of these cells to the precise location of defects. The research is aimed also to demonstrate (via collaborative efforts with another institution) the lack of any cancer promoting ability in these cultured cells. Future Research We intend to optimize the expansion of marrow-derived mesenchymal (stromal) stem cells and to demonstrate their immune modulatory properties to allow future cellular therapy for Inflammatory Bowel Diseases: Ulcerative Colitis and Crohns Disease. Once this goal is reached, research could expand to other immune-related diseases. Funding Needs So far, all research has been funded from internal sources. Stem cell transplantation study for the treatment of ALS Phase 2 The phase 2 trial focused on the safety and maximum tolerated dose of Human Spinal Cord Derived Neural Stem Cell Transplantation. This study expanded on the work of the phase 1 study directed by Eva L. Feldman, M.D., Ph.D., who is the principal investigator and director of the first-ever FDA-approved human clinical trial of stem cells injected directly into the spinal cords of ALS patients. Phase 1 of the trial, designed to study the safety of the procedure, was completed in 2013 with no significant adverse side effects to patients. And follow-up patient evaluations have produced some extraordinary data: Several participants in the trial, who were treated early in their disease, were determined to have had little or no significant progression of ALS for more than 700 days post-surgery. Updated July 2015: Fifteen patients were studied in the phase 2 study, the last 3 of which received 8 million stem cells injected into the lumbar spinal cord followed by 8 million stem cells injected into the cervical spinal cord. My wife made a comment last week that Ive thought about a lot since she said it. It was said in passing in the middle of a conversation that I cannot remember (apparently, my wifes not the only person in our family with poor memory). In this case, though, I think I fail to remember the rest of the conversation because her comment stunned me. I dont think Ill live a long life. I knew if I talked to her about it then, I would become too assertive about how wrong I thought the statement was and that she had no right to think that way. (Actually, she has the right to think any way she wishes and usually does even if I dont like it!) I knew, though, I wouldnt be able to listen to what she was really saying. What are hematopoietic cell transplantation (HCT) and peripheral blood stem cell transplantation (PBSCT)? HCT and PBSCT are procedures that use stem cells to treat a patient's malignancy or to repair diseased or defective bone marrow. A patient receives intensive chemotherapy with or without total body irradiation therapy in an attempt to kill all cancerous cells, but which also destroy his/her own bone marrow function. This therapy also causes immunosuppression, which prevents rejection of the newly transplanted stem cells from a related or unrelated donor. There is little risk of rejection of a patient's own stem cells following autologous transplant. After transplantation, the new stem cells replace the damaged bone marrow and cells of the immune system. HCT and PBSCT allow a patient to receive very high doses of chemotherapy and radiation designed to kill cancer cells. The high doses of therapy lead to the destruction of a patient's own marrow and immune system, which is then replaced by marrow from a donor or from peripheral blood stem cells that have been harvested before therapy. J Neurol Neurosurg Psychiatry. 2003 Sep; 74(9): 12211224. Background: Gluten ataxia is an immune mediated disease, part of the spectrum of gluten sensitivity, and accounts for up to 40% of cases of idiopathic sporadic ataxia. No systematic study of the effect of gluten-free diet on gluten ataxia has ever been undertaken. Objective: To study the effect of gluten-free diet on patients presenting with ataxia caused by gluten sensitivity. Methods: 43 patients with gluten ataxia were studied. All were offered a gluten-free diet and monitored every six months. All patients underwent a battery of tests to assess their ataxia at baseline and after one year on diet. Twenty six patients (treatment group) adhered to the gluten-free diet and had evidence of elimination of antigliadin antibodies by one year. Fourteen patients refused the diet (control group). Three patients had persistently raised antigliadin antibodies despite adherence to the diet and were therefore excluded from the analysis. Results: After one year there was improvement in ataxia reflected in all of the ataxia tests in the treatment group. This was significant when compared with the control group.
