Doctor disciplined for methadone prescriptions likely to close practice

GRUNDY CENTER, Iowa --- A doctor here will likely close his practice as a result of Iowa Board of Medicine disciplinary action against him in connection with his prescription of methadone to patients, his attorney said Tuesday. Kevin Engels, a Cedar Falls attorney representing Dr. Frank Lamp, said his client is disappointed in the states action. After discussing the matter with him, he will likely close his practice in the near future, Engels said. It comes a month after he was acquitted of related misdemeanor criminal charges. In addition to the methadone-related incidents, Lamp also was disciplined for performing an inappropriate and/or medically unnecessary physical examination of a young female patient at his clinic in 2011, for which Engels noted he was never criminally charged. Lamp had been accused before the state board of professional incompetency; inappropriate prescribing; improper pain management; violating laws and rules of the practice of medicine and unethical or unprofessional conduct. The Board of Medicine cited Lamp for professional incompetency and violating rules of the practice of medicine when he inappropriately prescribed controlled substances, particularly methodone, to patients with known substance abuse histories between 2009 and 2011. One change added several drugs to the list of those that may decrease phenytoin levels. Nevertheless, significant labeling remains intact. Although Dilantin has been FDA-approved since the mid-1950s, a couple of increased risks noted on Dilantins 21st century labeling have not changed. One pertains to the drugs fetal effect and it reads, If you take Dilantin during pregnancy, your baby is at risk for serious birth defects. Infant breast feeding is not recommended for women taking this drug because phenytoin appears to be secreted in low concentrations in human milk. Expounding on this warning, The babies of women taking phenytoin have a greater than usual number of major birth defects like cleft lip, cleft palate, and heart malformations, according to the Epilepsy Foundations Epilepsy Therapy Project. If they become pregnant, Dilantin patients should be encouraged by their physicians to enroll in the North American Antiepileptic Drug Pregnancy Registry, according to labeling in effect since 2009. It seems that acetaminophen is not only an effective pain reliever but also has subtle cognitive effects on those who take the drug which altered the way subjects passed moral judgments. What does this mean? It seems that acetaminophen takes the edge off of peoples anxiety. While this can generally be viewed as a positive, one of the studies raised novel legal issues. One of the ways in which the study was carried out was by testing how high Tylenol users set bond for a fictional prostitute against a control group who did not take acetaminophen. Those who were acetaminophen users set lower bail numbers which was alleged to be a function of their sense of moral judgment [being] blunted. (Perhaps even more telling the acetaminophen users seemed not to be so bothered by creepy, surrealistic David Lynch films.) If the claims about acetaminophen having a cognitive effect prove true, whether a potential juror is taking the drug would seem to be a necessary inquiry during voir dire. This ruling comes as an attempt to remedy the growing prescription drug abuse problem sweeping the nation. King County to address substance abuse as an illness

April 24, 2013 Vol: 20 No: 17 by: Aaron Burkhalter , Staff Reporter King Countys Mental Health, Chemical Abuseand Dependency Services Division is changing how treatment providers view those with chemical addictions. Before, a relapse into drug use was a failure in the eyes of King Countys treatment services. Now, its just another step in the process. Assistant Division Director Jean Robertson said recovery operates on a spectrum. Thats why the county, and the many organizations contracted to provide treatment, will now treat chemical dependency the same as mental health as a lifelong chronic illness that requires ongoing treatment. This shift will change how the county talks about clients and trains providers. The agency is also exploring the use of more peer counseling services, meaning people who have recovered from addiction are helping the clients as well. This model is already used for mental health treatment. This will guide how the county thinks about and engages with clients but wont create new funding for chemical dependency.
