Balancing oxaliplatin dose with neurological side effects in metastatic colon cancer

However, "Many patients don't receive the necessary dose to try to keep their cancer in check, because their symptoms become too debilitating and their quality of life is reduced," says Andrew Weickhardt, MD, clinical fellow at the University of Colorado Cancer Center. Weickhardt, along with Keith Wells, MD, and Wells Messersmith, MD, CU Cancer Center investigator and developmental therapeutics program co-leader, recently published a review in the Journal of Oncology that describes how to maximize the positives of this tricky balance between oxaliplatin's effects and side effects. "Before, when patients died in a few months, these symptoms were overlooked. Now they're living two to three years and the symptoms deserve closer attention," Weickhardt says. The review recommends two approaches, used alone or simultaneously – what Weickhardt calls a "stop and go" approach, along with supplements that may mitigate the drug's neurological damage. In the stop and go approach, a patient starts oxaliplatin along with another drug and takes both for two months. Then, the patient takes a hiatus from oxaliplatin (while continuing the first drug), and reintroduces oxaliplatin two months later or at the point of disease progression, whichever is sooner. The discovery, reported in the Feb. 2 Science,suggests that there are inherited but conquerable risk factors involved in drug dependency. What’s more, looking at how non-using siblings compensate for their inherited brain irregularities may eventually uncover clues to treating drug addiction. “These brothers and sisters might show us strategies that we could teach to drug users,” says Karen Ersche of the University of Cambridge in England and lead author of the study. It’s well known that people with drug addiction problems have irregular features in parts of the brain associated with self-control. Until now, it was unclear whether these abnormalities emerge after long-term drug abuse or if some people are born with brains that are more susceptible to drug addiction. BOSTON (Reuters) - Ronald O'Hanley has emerged as an outspoken force inside Fidelity Investments, giving a speech on Thursday that touched on sex, drugs and revolution as he warned that young Americans face getting a raw deal from the U.S. retirement system. Top executives at the Boston-based mutual fund giant are known for being conservative and scripted in their public appearances. But since O'Hanley joined Fidelity in 2010 as the president of asset management and corporate services, he has demonstrated a more free-wheeling approach in his public comments. The United States is facing a "real crisis in intergenerational inequity," O'Hanley said in explaining how Social Security's cash deficits continue to grow as retirees outnumber the people entering the workplace. The crucial question that faces O'Hanley's college-age son and his friends, he said, is what kind of retirement system they will have when they grow old. The United States is at the tipping point in dealing with problems affecting the retirement savings system, O'Hanley said.
