Cardiology 2016 | The Children’s Hospital of Philadelphia

Last February, more than 800 pediatric cardiologists, intensivists, surgeons, nurses, and others involved in the care of patients with pediatric and congenital cardiovascular disease descended on Scottsdale, AZ for the 18th Annual Update on Pediatric and Congenital Cardiovascular Disease. Throughout the conferences various presentations and panel discussions, attendees were encouraged to actively participate in the conversation with Q&A sessions. Lex Perri, MSN, RN, NE-BC, director of Cardiac Center Nursing at The Childrens Hospital of Philadelphia and Cardiology 2015 faculty member, listens intently in one of the many nursing sessions offered to attendees. What are hormones? Hormones are substances that function as chemical messengers in the body. They affect the actions of cells and tissues at various locations in the body, often reaching their targets through the bloodstream. The hormones estrogen and progesterone are produced by the ovaries in premenopausal women and by some other tissues, including fat and skin, in both premenopausal and postmenopausal women. Estrogen promotes the development and maintenance of female sex characteristics and the growth of long bones. Progesterone plays a role in the menstrual cycle and pregnancy. Estrogen and progesterone can also promote the growth of some breast cancers, which are called hormone-sensitive (or hormone-dependent) breast cancers. How do hormones stimulate the growth of breast cancer? Hormone-sensitive breast cancer cells contain proteins known as hormone receptors that become activated when hormones bind to them. The activated receptors cause changes in the expression of specific genes, which can lead to the stimulation of cell growth. To determine whether breast cancer cells contain hormone receptors, doctors test samples of tumor tissue that have been removed by surgery. If the tumor cells contain estrogen receptors, the cancer is called estrogen receptor-positive (ER-positive), estrogen-sensitive, or estrogen-responsive. Ulcerative colitis may be suspected when a person has experienced symptoms of rectal bleeding, intermittent diarrhoea and abdominal pain. As part of the diagnosis the doctor will take a full medical history and conduct a physical examination. The doctor may request that blood tests and specimens of the bowel motion are taken. In ulcerative colitis, blood test results will often indicate anaemia and signs of inflammation in the body. Samples of bowel motions will often indicate the presence of blood, pus and mucous. If ulcerative colitis is suspected,endoscopy may be recommended. Endoscopy is the most important diagnostic test used to diagnose ulcerative colitis. During this test a small flexible tube (an endoscope) with a fibre-optic camera at its tip is passed into the rectum and colon. The doctor is able to see the lining of the rectum and colon on a television screen and can look for signs of inflammation and ulceration that may indicate ulcerative colitis. Small tissue samples (biopsies) from the lining of the colon and rectum can be taken for testing. Ulcerative colitis can be diagnosed by the characteristic abnormalities of this tissue. HHS takes steps to provide more information about clinical trials to the public In an effort to make information about clinical trials widely available to the public, the U.S. Department of Health and Human Services (HHS) has issued a final rule specifying the requirements for registering and reporting summary results information to ClinicalTrials.gov. The new rule expands the legal requirements for submitting results. NIH also has issued a complementary policy for submitting summary results information for all NIH-funded clinical trials. New NIH grants will fund developing computational approaches to finding the differences in DNA that make people susceptible to disease. The awards, totaling around $11.1 million, support research in identifying these differences - or genetic variants - in the less-studied regions of the genome. Understanding these variants should provide clues to understanding how disease develops. NHGRI has found a new syndrome characterized by intellectual disability, hearing loss, abnormal sexual development and birth defects. It is caused bynew mutations inCHD4, which regulates how DNA is packaged and how RNA copies gene sequences during development. Children and adults with cerebral palsy require long-term care with a medical care team. This team may include: Medications that can lessen the tightness of muscles may be used to improve functional abilities, treat pain and manage complications related to spasticity or other cerebral palsy symptoms. It's important to talk about drug treatment risks with your doctor and discuss whether medical treatment is appropriate for your child's needs. Medication selection depends on whether the problem affects only certain muscles (isolated) or the whole body (generalized). Drug treatments may include the following: Isolated spasticity. When spasticity is isolated to one muscle group, your doctor may recommend onabotulinumtoxinA (Botox) injections directly into the muscle, nerve or both. Botox injections may help to improve drooling. Your child will need injections about every three months. Side effects may include pain, mild flu-like symptoms, bruising or severe weakness. Other more-serious side effects include difficulty breathing and swallowing. Generalized spasticity. If the whole body is affected, oral muscle relaxants may relax stiff, contracted muscles. These drugs include diazepam (Valium), dantrolene (Dantrium) and baclofen (Gablofen). Diazepam carries some dependency risk, so it's not recommended for long-term use.
